Good Shepherd Hospice is seeking an experienced hospice LPN to serve our patients and families in OLATHE and surrounding communities. 

The LPN Lead Weekend Nurse at Good Shepherd Hospice based out of our OLATHE, KS office plays a crucial role in ensuring the delivery of high-quality patient care over the weekends. This position requires a dedicated and compassionate individual who is licensed as a LPN and possesses strong leadership skills. The Full Time LPN Lead Weekend Nurse oversees clinical operations, coordinates patient care activities, and provides support to the interdisciplinary team, ensuring continuity and excellence in care provision.

Responsibilities

1. Clinical Leadership:
    - Supervise and coordinate clinical activities during weekend shifts.
    - Provide leadership and guidance to nursing staff, ensuring adherence to policies, procedures, and regulatory requirements.
    - Monitor patient care delivery and intervene as necessary to maintain quality and safety standards.
    - Collaborate with the interdisciplinary team to develop and implement care plans tailored to individual patient needs.

2. Patient Care Coordination:
    - Assess patient needs and collaborate with the care team to develop comprehensive care plans.
    - Coordinate and oversee the delivery of nursing care services to patients and families.
    - Conduct regular assessments to evaluate patient progress and adjust care plans accordingly.
    - Serve as a liaison between patients, families, and the interdisciplinary team to facilitate communication and ensure holistic care delivery.

3. Staff Support and Development:
    - Provide guidance, mentoring, and support to nursing staff, fostering a positive and collaborative work environment.
    - Conduct regular performance evaluations and provide constructive feedback to staff members.
    - Facilitate training and continuing education opportunities for nursing staff to enhance their skills and knowledge.

4. Documentation and Compliance:
    - Ensure accurate and timely documentation of patient assessments, care plans, and interventions in accordance with organizational standards and
regulatory requirements.
    - Monitor compliance with infection control protocols, safety regulations, and accreditation standards.
    - Participate in quality improvement initiatives and assist in the implementation of best practices to enhance patient care outcomes.

5. Communication and Collaboration:
    - Maintain open lines of communication with the interdisciplinary team, including physicians, nurses, social workers, chaplains, and other healthcare professionals.
    - Participate in team meetings, case conferences, and other collaborative forums to discuss patient care plans and address any concerns or challenges.
    - Communicate effectively with patients and families, providing education, emotional support, and assistance in navigating the hospice care journey.

Qualifications

  • Valid Nursing License
  • Hospice Experience Required
  • Strong organizational and self-management skills
  • Valid driver’s license
  • Must be of high integrity including maintenance of confidential information including all patient records 
  • Works as a team player, following directions and adhering to defined care plans 
  • Demonstrated commitment to the hospice philosophy of care
  • Can successfully pass background checks
