- Coordinate and oversee the delivery of nursing care services to patients and families.
- Conduct regular assessments to evaluate patient progress and adjust care plans accordingly.
- Serve as a liaison between patients, families, and the interdisciplinary team to facilitate communication and ensure holistic care delivery.
3. Staff Support and Development:
- Provide guidance, mentoring, and support to nursing staff, fostering a positive and collaborative work environment.
- Conduct regular performance evaluations and provide constructive feedback to staff members.
- Facilitate training and continuing education opportunities for nursing staff to enhance their skills and knowledge.
4. Documentation and Compliance:
- Ensure accurate and timely documentation of patient assessments, care plans, and interventions in accordance with organizational standards and
regulatory requirements.
- Monitor compliance with infection control protocols, safety regulations, and accreditation standards.
- Participate in quality improvement initiatives and assist in the implementation of best practices to enhance patient care outcomes.
5. Communication and Collaboration:
- Maintain open lines of communication with the interdisciplinary team, including physicians, nurses, social workers, chaplains, and other healthcare professionals.
- Participate in team meetings, case conferences, and other collaborative forums to discuss patient care plans and address any concerns or challenges.
- Communicate effectively with patients and families, providing education, emotional support, and assistance in navigating the hospice care journey.
